Overview

Recently, WHO proposed a change to the classification of pre-diabetes to intermediate hyperglycaemia (IH; [Bergman, 2024]) in order to avoid any stigma associated with the diabetes diagnosis and to recognise that, with interventions, it may be reversed [Tuomilehto, 2019]. Early and accurate diagnosing of IH is key so that high-risk individuals can be referred for intensive lifestyle modification and treatment to prevent progression to type 2 diabetes [Bergman, 2020]. The evidence supports that IH is a serious dysfunction which should be diagnosed as early as possible to help prevent its transition to type 2 diabetes [Elgart, 2023; Lawal, 2020; Richter, 2018]. The current criteria for diagnosis, based on glucose thresholds and glycated haemoglobin (HbA1c) have limitations [Zhang, 2023].  Guidelines for screening for pre-diabetes/IH or type 2 diabetes state that it should be considered in asymptomatic adults who are overweight or obese or who have one or more risk factors [ADA, 2024]. However, the reliance on fasting plasma glucose (FPG)/HbA1c alone can lead to inaccurate diagnosis and/or missed opportunities to manage IH before it progresses to type 2 diabetes [Bergman, 2024].  The recent International Diabetes Federation (IDF) Position Statement [Bergman, 2024], reviews findings that individuals with 1-h plasma glucose (PG) ≥155mg/dL (8.6mmol/L) are considered to have IH and those with 1-h PG ≥209mg/dL (11.6mmol/L) are considered to have type 2 diabetes. The 1-h PG test has been shown to improve diagnosis of IH and predicting type 2 diabetes in individuals earlier and more efficiently than the 2-h PG thresholds [Alyass, 2015; Peddinti, 2019]. The summary of the 40-year evidence in the IDF Position Statement supports changing the current screening and diagnostic recommendations for IH and type 2 diabetes [Bergman, 2024].

Martin Savage is Emeritus Professor of Paediatric Endocrinology at William Harvey Research Institute, Barts and the London School of Medicine & Dentistry, Queen Mary, University of London. He was head of the Paediatric Endocrine Unit at Barts and the London School of Medicine from 1982 to 2007. He has interests in growth disorders, specifically those with abnormalities in the GH-IGF-1 axis and in phenotype-genotype relationships of GH-IGF-1 axis defects, notably GH resistance. He published the first human case of an IGF-1 gene defect in the New England Journal of Medicine in 1996.  His other interests are Cushing’s syndrome and growth in chronic inflammatory diseases. He was General Secretary of the European Society for Paediatric Endocrinology (ESPE) from 1997 to 2004. He has lectured in 60 countries and has published 482 original articles, reviews, textbook chapters and books. In 2007, he was awarded the ESPE Andrea Prader Prize for contributions to paediatric endocrinology and in 2018 he received a Visionary Award from the American Human Growth Foundation. In 2022, he received a Research Excellence Award from the Dr Sulaiman Al Habib Medical Journal in Riyadh, and the British Society of Paediatric Endocrinology & Diabetes James M. Tanner Lifetime Achievement Award. He continues to lecture nationally and internationally.

Michael Bergman was awarded the MD degree cum laude from the Catholic University of Louvain (Belgium) and trained in internal medicine at the University of Maryland and then completed a fellowship in endocrinology, diabetes and metabolism at Yale University, Columbia University and the Albert Einstein College of Medicine. He is Professor of Medicine and Population Health at the NYU Grossman School of Medicine and Director of the NYU Diabetes Prevention Program. He also serves as Section Chief, Holman Division of Endocrinology, Diabetes, Metabolism at the VA New York Harbor Healthcare System where he developed a Diabetes Prevention Clinic for veterans. He has published and lectured widely on prediabetes and diabetes prevention including editing two monographs and a textbook. He has participated in international collaborations describing the 1-hour post-load glucose as a more sensitive marker for detecting individuals at high-risk for progression to type 2 diabetes in various populations. He co-chaired an expert panel formulating the International Diabetes Federation Position Statement on the 1-hour post-load plasma glucose for the diagnosis of intermediate hyperglycaemia and type 2 diabetes.

Antonio Ceriello is Research Consultant at IRCCS MultiMedica, Milan, Italy, Adjunt Professor of Internal Medicine at Luigi Vanvitelli University of Naples, Italy and Guang Ci Professor at Ruijin Hospital, Shanghai Jiao Tong University School of Medicine, China. Professor Ceriello has published widely in the area of diabetes with over 500 original papers and with an h-index of 110 (SCOPUS). He chaired the International Diabetes Federation (IDF) committee for the development in 2008, and in the 2011 for the update of “Guideline for Management of Postmeal Glucose ”. Currently, is the President of the European Association for the Study of Diabetes (EASD) Study Group on “Diabetes and Cardiovascular Diseases”. He also served as Board member of the “2019 European Society of Cardiology (ESC) guidelines on diabetes, pre-diabetes, and cardiovascular diseases developed in collaboration with the EASD “. He also served as Associate Editor of “Diabetes Care” from 2003 to 2011 and Associate Editor of “Diabetic Medicine” from 2005 to 2016. From July 1st 2016 he has been appointed as Editor in Chief of “Diabetes Research and Clinical Practice”, the official IDF journal. Prof. Ceriello has also been appointed by IDF as Co-Chairman for the development of the “2025 Global Clinical Practice Recommendations for the Management of Type 2 Diabetes” and as Chairman for the 2025 IDF World Congress, in 2025 by EASD as Member of the Committee on Clinical Affairs (CCA) for 2025-2028 and delegate as representative of EASD at European Medical Agency (EMA). Prof. Ceriello is the winner of the 2004 “Camillo Golgi Prize” of the EASD, of the 2017 of the “Hellmut Mehnert Award”, during the IDF 2017 Meeting, in the 2022 of the “Aretaeus Lecture Award” from the Hellenic Society of Diabetology, in 2023 of the “Somogyi Award” from the Hungarian Diabetes Association and in of the 2024 “Shaukat Sadikot Oration” at Diabetes India Congress.

Dr. Abboskhujaeva Lola is Chairperson of the Endocrinological and Diabetes Association of Uzbekistan (EDAU) and Grant Manager since 2022.

After graduating in general medicine in 2006, she went on to earn a Masters degree in Endocrinology, Tashkent Medical Academy, Tashkent State Medical Institute. Following this, she was a post-graduate student and an endocrinologist at the Center for the Scientific and Clinical Study of Endocrinology, Uzbekistan Public Health Ministry until 2015 and then a researcher at the Diabetology Laboratory until her current position.

Andres V. Ebison Jr is a practicing Clinical Endocrinologist in Metro Laguna, south of Manila, Philippines. He took his residency in Internal Medicine and Fellowship in Endocrinology, Diabetes and Metabolism at the Makati Medical Center. He is a Diplomate and Fellow of the Philippine College of Physicians and the Philippine College of Endocrinology, Diabetes and Metabolism (PCEDM). He currently sits as one of the Trustees of PCEDM and co-chairs several committees (Advocacy, Training) under the college. He is also part of the Philippine Center for Diabetes Education Foundation Inc as one of its faculty in its medical bureau and Camp COPE (Camp for Teens with Type 1 Diabetes Mellitus) and Camp HOPE (Camp for couples with Type 2 Diabetes Mellitus) Program Director. He is involved in the academy as an Assistant Professor at the University of Perpetual Help System Laguna College of Medicine and is Deputy Medical Director at Westlake Medical Center.

Dr. Banshi Saboo is the Chief Diabetologist & Chairman at the Diabetes Care & Hormone Clinic, Ahmedabad; Chair-Elect, International Diabetes Federation (IDF) South East Asia; Vice-president, Diabetes in Asia Study Group (DASG); Secretary, Diabetes India & Indian Academy of Diabetes (IAD) and founder of D-Tech, Diabetes Care India (NGO), India SWEET Registr. Prior to this, he was President of the Research Society for the Study of Diabetes in India (RSSDI) and  All India Association for Advancing Research in Obesity (AIAARO).

He is a member of the European Association for the Study of Diabetes (EASD) Global Advisory Council and a member of the American Diabetes Association (ADA) Advisory group and holds fellowships with American College of Endocrinology (USA), American College of Physicians (USA), International College of Nutrition (FICN), Royal College of Physicians (Glasgow, Edinburgh, London) and National Academy of Medical Sciences (MNAMS; Diabetology).

Dr Saboo has authored more than 150 journal publications and book chapters and holds Editorial positions as Managing Editor (Sadikot’s International Textbook of Diabetes & Diabetes & Metabolic Syndrome: Clinical Research & Reviews), Editor-in-chief (RSSDI Textbook of Diabetes) and Associate Editor for the official journal of IDF, (Diabetes Research & Clinical Practice).

He is an expert in the RSSDI Guidelines for Management of Diabetes, ATTD CGM Guidelines & ISPAD Guidelines for Paediatric Diabetes and initiator of the RSSDI-Defeat Diabetes Campaign (with a screening world record of 1.2 million on World Diabetes Day) and has been awarded the Most Impactful Healthcare Leader by the World Health and Wellness Congress and the Global Clinician of the Year (2024), by the American College of Clinical Endocrinology (AAACE).

Medical specialist in Endocrinology, Master and Doctor in Medicine from the Universidad Peruana Cayetano Heredia. Master in Epidemiology and Public Health and Doctoral Studies in Geriatrics and Gerontology, Complutense University of Madrid, Spain.  Master’s studies in Political Science from the Pontifical Catholic University of Peru. During the last 40 years as a teacher and researcher at the Universidad Peruana Cayetano Heredia, he conducted epidemiological studies of chronic diseases causing cardiovascular disease such as diabetes mellitus, hypertension, obesity, dyslipidaemia and metabolic syndrome.  His research has shown that the incidence of type 1 diabetes in children under 15 years of age in Peru is one of the lowest in the world. In 2015, the first national diabetes prevalence study (PERUDIAB) was completed, demonstrating that 7 out of every 100 Peruvians over 25 years of age have diabetes mellitus and that 23% also have fasting hyperglycaemia (prediabetes), which affects 6 million Peruvians. He has contributed in Peru, to the promulgation of the Law for the Protection of People with Diabetes and its recent regulations in May 2023, as well as the clinical practice guidelines in type 1 diabetes. He is currently directing the Changing Diabetes in Children (CDiC) Program on Type 1 Diabetes in the primary level health network of the Diris Lima Sur and Lima Norte, having completed an epidemiological study of Gestational Diabetes by the World Diabetes Foundation (WDF).

Dr. Zulaykho Shamansurova (MD, PhD, DSc) is Associate Professor at the School of Medicine, Central Asian University, Uzbekistan (since 2022). Prior to this, she was Associate Professor at Tashkent Pediatric Medical Institute, Uzbekistan.

Dr. Shamansurova has considerable research experience in endocrinology with posts as Consultant Endocrinlogist/Principal Investigator at the Research Institute of Endocrinology, and Institute of Biochemistry, Tashkent, Ubekistan. Between 2011–2018, she was Doctoral Research Student and then Research Assistant at Montreal University and Montreal Research Academy (Quebec, Canada), respectively.  

She is an active member of the Endocrinology and Diabetes Association of Uzbekistan and previous Editor of Diabetes in Uzbekistan Journal (2005–2007). She was an active participant in several International Diabetes Federation (IDF) conferences and was a contributor to projects such as, ‘Insulin for Life’ and ‘Life for a Child’ in Uzbekistan.
