Overview

Many strategies to improve implementation of hypertension guideline recommendations have been proposed [Whelton, 2023]. Additionally, the use of monotherapy to treat hypertension may be evolving [Jackson, 2023; Cooper-DeHoff, 2021] as access to effective and affordable single pill antihypertensive combination drug therapies forms part of the latest ESH guidance [Mancia, 2023; Whelton, 2023] and combination therapies are increasingly effective in lowering BP compared with monotherapies [Jackson, 2023; Wald, 2009]. Accurate BP measurements with home or ambulatory monitoring is a key recommendation for the primary prevention of hypertension which are also highlighted in US (American College of Cardiology, American Heart Association) and European (ESH and ESC) guidelines  [Whelton, 2023; Sorte, 2023]. Additionally, the importance of identifying and treating nocturnal hypertension (BP ≥120/70 mmHg) is highlighted in the ESH 2023 guidelines [Mancia, 2023]. Multi-component primary care interventions for hypertension have had some recent success in reducing cardiovascular disease (CVD) incidence, morbidity and mortality [Chay, 2024]. For this intervention to effectively operate, factors such as capacity, leadership and sufficient external support play an important role [Hespe, 2022]. Further ways to deliver interventions are being considered [Maroney, 2023]. Non-medical interventions, such as the low-calorie Dietary Approaches to Stop Hypertension [DASH] diet, have proven to be effective in reducing CVD risk factors [Le Goff, 2023]. Combining this diet along with other lifestyle changes, effectively reduced CVD risk factors [Whelton, 2023; Mancia, 2023].
